 Dear PHCA Parents,

Please review and follow the below steps in completing the paperwork attached below.  Print the "All Forms Bundle" or download/print one at a time.

 All paperwork needs to be printed and completed and submitted prior to or at the PHCA Parent Orientation on August 28th.

Description  Instructions
Health Summary FormInformation on allergies and health conditions    Submit one form for each new student or if your student's health conditions have changed
Transportation Departure FormIndicate how your student will leave school at the end of each day.
For students in K-3rd grade, this information will be placed on a tag for their backpack.    
Bus info is supplied by the bus companies via post card.  Look for this info in your mail.
One form for each student K-3rd grade.
Immunization Form        Each student must have an accurate copy on file.One form per new student and updated for 7th graders.
Lunch Program Details        Instructions and Sept Menus/Forms coming soon.
Uniform Orders    Uniform Ordering will be online from Aug 28 to Sept 8th.optional
